Possession - Section 8 and Section 21

I have a tenant whose AST agreement states taht she has to pay monthly. However, she has been paying weekly. Now I want rid of her and have come to filling in the Section 21 Notice, do I use a monthly method of filling it in or do I break it down into weekly?

Plus, if anyone could shed light on how to fill in a section 21 for weekly payers I would be grateful.
