Possession - Section 8 and Section 21

I have a tenant whose AST started on 31/10/2012. I wish to serve a S21(4)a. I will serve it on 29/12/2012 but may have to put it through her letterbox if she isnt there.
Serving the notice will be witnessed.

1. Do i enter the date as 30/12/2012in case she isn't in.
2. Where the form states" After .... " do I state 28/2/ 2013. Obviously there isn't a 30th of Feb but the 28/2/2013 would normally be when her rent was due.
