Prescribed information - do I have the right form/details?

My tenants' deposits are all protected with the DPS. When they were first protected in 2007 I purchased 'protecting your deposit' forms from a well known legal stationery firm and issued this to each tenant. The form shows the name of the scheme I used (i.e. the DPS), deposit amount, property details, tenants details and my details and circumstances in which the deposit maybe retained by the landlord etc. I attached to it a copy of the DPS's terms and conditions (all 6 pages)which was also given to the tenant. Each form was then signed/certified by me and my tenant.
I am worried that this may not be deemed the correct format for the 'prescribed information' that has to be given to tenants. Can you clarify if this is OK? Thanks
