Proof of service and method of service

Good afternoon

I am curerntly filling in an N5B form and am struggling with the wording to use for method of service.

I posted the notice through the tenants door with my col. as witness. What method is this classed as?
Also what do I use for proof of service, I thought that the court service had a form for this but I can not find it

Any ideas
