Please could you advise me on the procedure for ending the above tenancy and the forms and notification needed.

1) Is a letter sufficient or should it be on a specific form?

2) should the date of the original agreement that has rolled on eg. 18th December be the date used for the expiry of the Section 21(4) or could 9th December be used as long as two months notice is given.
