Break Clauses in Tenancy Agreement

I want to sign a tenant to a 12 month tenancy but would be keen to have a break clause to be effective at the 6 month stage for either the landlord or tenant to end the tenancy.

Could any standard break clause wording be provided and any advice on effectiveness of such clauses.
