Correct forms and procedure for S21 Notice

Hello, I need to give my tenants notice and I want to make sure I do this the right way.

Their rent is due on the 26th of each month.
Their current AST term 'expired' on the 25th May 2011 and has rolled as a monthly contractual agreement since then.
They have been great tenants so I want to be courteous.

Please will you confirm which S21 form I should use and what dates I should serve the Notice? Is it simpler to serve it on the 25th or 26th August?
