Tenancy Deposits

Hello,

My tenant's six-month shorthold assured is soon becoming a statuatory periodic tenancy so, in the light of the Superstrike situation, I would like to re-issue the PI. I have downloaded the PI template from the DPS and I have a couple of questions about it.

1) Para 7d asks for a tenant contact address to be used by the Landlord at the end of the tenancy. What should my tenant put here as she is obviously in the flat we have rented to her and has no other contact address to give? Should she just put the flat's address?

2) In providing DPS with the date of the start of the Statuatory periodic tenancy, is it correct to use the date six months since the Shorthold tenancy started - her tenancy started on April 1st 2014?

Also, I understand that, after my having informed and agreed action with the tenant, the custodial deposit needs to be withdrawn and resubmitted to the DPS. Does the deposit have to withdrawn by my tenant and resubmitted by her or can I just withdraw it and resubmit it myself, please?
