How to fill out a Form 4 / Section 13 notice for increase of rent

I am a little unsure as to how to complete section 5 of the form. Guidance note 12 gives some guidance, but I remain unsure as to what to put in the boxes when the tenant pays the council tax directly to the council, the water charges directly to the water company and there are no fixed service charges. If I put "nil" for these, does it imply that the council tax, for example, is included in the rent? Any comments would be appreciated.
