HMRC Requirements for Form 17

I own a rental property as Tenants in Common with my husband in unequal shares. We are therefore wishing to register/notify HMRC accordingly using the Form 17. HMRC requires additional information such as a declaration, but I could find no guidance as to what would constitute an acceptable declaration to HMRC. Specifically, I am wondering:

(1) Is there some HMRC guidance out there somewhere I've not found?
(2) Is there a template for a suitable declaration out there to allow me to do this at no/low cost?
(3) Does the declaration have to be witnessed/signed by something like a notary public?

Any advice or thoughts gratefully received.
