Direct rent payement arrears form

We are trying to complete the Direct Rent Payement Arrears Government form but we are unable to provide the date of birth of the tenant as when the tenant moved in 14 years ago this was not required.
Could you please advise us on how to apply for the rent arrears when not able to supply this information.
