Possession - Section 8 and Section 21

hi, i have a long standing tenant. due to a change in our personal financial situation i need the property, currently let, as my residence. The 6 month AST started on 12/12/2009 and has been running as a periodic since. I have filled in form 6a but it does not show a leave date, which was 2 months hence on their contract. I understand that covid changes extended the 2 months to 3, and also that the form should clearly show the expected day/date of the tenants departure.
Have i got the correct form for this age on contract? if so where do i insert the leave date?
