GUARANTOR FORM MISSING INFORMATION

The document sent by the letting agent is incomplete as there is missing information, specifically the

  1. Date of the Guarantor Agreement;
  2. The Parties being The Landlord/ Agent and The Tenant;
  3. The Date of the Tenancy Agreement to which the Guarantor Agreement refers.

The form has been signed by the Guarantor and witnessed by the letting agent. Despite having sent repeated requests to the agent requesting they email me scanned copies of the signed agreement with the completed information where missing. So far they have not complied with my request. Is the document still admissible?
The tenant is now in arrears and I had intended to serve notice on the Guarantor to clear the arrears.
