Possession - Section 8 and Section 21

If a landlord is living abroad, what address should be put on the Section 21 notice as the landlords address?

The tenant is a mother and son. The son will turn 18 during the tenancy. Should he be named as a tenant? Should both mother and sons' name appear on the Section 21 Notice?
