Possession - Section 8 and Section 21

I have a tenant via the council since Jan 2010. In March, I started receiving complaints from Management Company for anti-social behaviour, smoking drugs in the communal area, intimating behaviour. To end it all, police raided the flat and arrested tenant on possession of drug and illegal weapon which has turned out to be stun gun. I wrote to the police and recieved confirmation from them. I served notice Section 8 grounds 14a and 14b in April and notice expired on 20th May. Tenant has refused to move out as she has been advised by the council that they should not hand over the keys until Landlord has court possession order.

I started completing N9B accelerated possession order. Made my way to the county court but I doubt whether I have completed the correct form (whether I should complete Form N5 instead of N5B). Please advise.
