How can i deliver a section 21 AND wil my original section 21 issued 2 years ago suffice

Hi
I wish to evict a tenant who has been in my property since 20th June 2006.
My tenant signed a section 21 notice on the same day ie. 20/6/2006 advising expiry date 19/12/06.

1.Can you please advise if I can use this old section 21 notice or do i have to issue a new one.

2. If I have to issue a new one (I believe this will be section 21 (4)(A). Can you advise if I can post through his door with a witness. (Will this suffice in court, and also would it be ok if this wittness is my father or does it have to be someone not related.

Please advise me on this as soon as possible.
