Section 21 - Break Clause

A Tenant has rented one of my properies on an Assured Shorthold Ternancy Agreement, there is a Break Clause in the Agreement, I am not happy with him as a Tenant and am applying to the local County Court for a Warrant of Possession.
The Tenancy Agreement was for 12 months and started on the 15/09/09 and the rent is paid 4 weekly, Can you inform me of which Section 21 Notice that i should issue him ???
