Requiring possetion

I have a tenant on assured shorthold tenacy which has since expired some 12 months ago. I want possetion of my flat. I served him with section 21 notice at the start of tenancy of which I have a signed copy.
Please advice the correct procdure for serving him with a notice to quit.
His tenancy started on 12 December 2005.
Your very quick response shall be highly apprichated considring the tenancy start date of tenancy.
