6 month break clause on a 12 month AST

I have been offerred a Incentive by the local council to extend an AST for a tenant on housing benefit who is affected by the LHA cap. The council say I must issue a 12 month AST to receive the incentive, but can include a 6 month break clause as protection if the tenant fails to pay during the life of the AST. What wording should I use to add this 6 month break clause to the standard RLA tenancy agreement.
