How to claim arrears from my tenant who has made no effort to repay them

My tenant is in arrears and has been for some time now. We have approached him regarding the matter and have suggested how he might get himself out of debt but to no avail. At the moment he is living rent free in my property. What form do I need to use in order to reclaim these arrears?
