Change to Statutory Periodic Contract

Our tenants are on an agency AST which we now realise does not mention automatic role on to Periodic or provision for rent increase.

It is ending shortly and the tenants wish to stay on on a rolling contract. How do we convert this to a Statutory Periodic Contract and where might we find the forms to do this? We assume we will need to do a Section 13 to increase the rent.
