Does NRLA or members have a list of useful clauses to add to the standard AST issued by my LA?

Just out of a lengthy eviction bad experience and before a new tenant signs on I would like to seek advice and expertise in terms of any clauses I may add:

Examples:

Pets or no pets, pet insurance, or permission

Registering property as a business address, or worse running a business without seeking permission

Minor snags and whose responsibility? e.g. a loose curtain fitting screw or replacing a light bulb/shower head.

Rent increases

Anything else that would be useful to add?
