Form 4 Rent Increase

I own a flat which was Short Term Fixed Tenancy, but has now gone periodic. I am trying to use Form 4 to notify of a rent increase, but am totally confused by Line 3 "the first rent increase date after 11 February 2003 is ..".
Can I ignore this and leave it blank based on the first exception quoted in Note 17.
