Possession - Section 8 and Section 21

I have a flat with a 6 month assured shorthold tennancy agreement. The tennant has been present for many years.

We raised the rent last year and signed a new agreement with an end date of Feb. 09. This has been allowed to roll over.

Damp has been identified and we need vacant possession to deal with this.

I want to confirm that the form I need to regain possession is 21(4)(A)? I'm looking for repossession next March.

Is there any other action I need to take before March?
