Tenants sub-letting

I have a fairly large 2 bed flat, original tenants a couple with small child and a friend. Friend moved out and couple kept tenancy, telling me that a 'cousin' would soon move in.
I told them that would be ok as long as I had name etc and that i would provide a written Permission to reside.
Now a year or so later they are asking for another such letter with different names. I visited the flat today for minor plumbing repairs and it seems they have moved several 'cousins' in.

They pay the rent on time every month. Flat is in an ok state...but to be honest if they moved out it would need a total update and redecoration.

Where do I stand legally?
Has this become a HMO and if I provide permission to reside am I potentially in hot water?

Should I ignore this and keep on collecting the rent...or do I need to step in and ask questions at the risk of losing paying tenants?
