Section 21 during fixed term

I am managing a property for a relation.

Tenant has 6 month fixed term AST start date 4th Jan ,end date 3rd July 2014.
Need to get this guy out.

Want to serve a Sec21 (1) (B) as it is during the fixed term

1. Can we serve it any date as long as it is before 3rd May?
2. If tenant is not at the property to sign our copy what else do we need to do,witness it being put through letterbox,photograph etc?
3. On the form what date do we put where it says "After" .? Do we put 3rd July?
4. If tenant refuses to leave what happens then with Court and Bailiffs? What action do we need to take?
